An interesting new study - systematic review and meta-analysis for ivermectin. Found: - no benefit for all-cause mortality - no benefit for length of stay Both very low-quality evidence https://www.medrxiv.org/content/10.1101/2021.05.21.21257595v2 …pic.twitter.com/RfEbosVA55

I haven't had time to read it in extreme detail yet, but a quick skim seems to show that it is a fairly good piece of research that the authors have already improved in the 24 hours since it went online

Arguably the most important point of the study - the vast majority of evidence on ivermectin for COVID-19 appears to be of extremely poor quality even when you limit the results only to the best studiespic.twitter.com/EjfJJR0uo3

I should say - it is yet another enormous, hideous black mark against our scientific nous during this pandemic that the evidence we have about a treatment that has been given to 100s of millions of people is "very low" quality Shameful, really

Even if ivermectin is useless, we should have more than a handful of awful trials about it And we don't

A quick skim in the results say that it doesn’t reduce mortality with a RR 0.37. I guess nothing is enough these days.

The results show no statistically significant results, but they also are from studies that are so low quality that even this finding is essentially meaningless

Isn't the point that it should not be used so widely based on a handful of awful trials? At this point, recruiting patients for a trial would be increasingly unethical.

Yes, but we're FIFTEEN MONTHS into this, and 100s of millions have been treated with ivermectin, and yet the best evidence we've got on whether it works is essentially just a big shrug

To be fair the real shame should fall onto all of those who knowingly pushed an unevidenced & potentially dangerous Tx on desperate survivors. I will never forget seeing survivors taking animal Invermectin & working out dose equivalents with no idea *shudders*

The main protagonists Kory & Marik produce some of least reliable evidence: historical controls from early 2020 11-12 drug treatments no randomisation or blinding V low quality evidence w high risks of bias

WHO & clinical trial specialist could easily design & oversee higher quality studies w relatively small funding outlays

Well, from the graphic you posted, it seems they used GRADE for quality appraisal, so all they could state that "more studies" would truly be needed to pass a final verdict one way or another.

Ethically, there should be enough positive evidence/data to test this out in a clinical trial. It is not good enough to conduct a trial just to disprove 'finally' a poor hypothesis. Patients should not be used like that. There are a lot of other medicines with greater rationale.

With the 40+ years of proven safety - do you think it reckless to continue to use ivermectin as a critical-care treatment by frontline critical care workers or would you endorse the continued use in case it turns out to be both safe and effective? I want to see a robust toolkit.
