TBF, that actually is the case for a lot of the research. E.g. I looked up Spengler, the highest OR study. It's a survey in Russia shortly after the fall of the USSR, with most dwellings built 1970s or earlier and 80%ish with no ventilation.

Hard to unpick here, one warning sign is that the ORs for symptoms (wheeze) are all essentially 1, but for doctor diagnosed asthma are huge. Suggests confounders at play to me.
