Worth noting that the expected rate of 1 is based on the very highest incidence rate described in the literature (most studies quote a 5th to a 3rd of that).

I've been thinking about this for a few days. 1.) If the threshold for action by government is explicitly based on a test of statistical significance (and it is) then it is out of our hands. We have to account for multiplicity.

2.) we have to account for multiplicity in terms of multiple groups as well as multiple endpoints because of extra-jurisdictional decisions. It's not just "how many disease" but "in how many biologically arbitrary groups"
