If willpower is indeed a useful concept, it must be an intervention that can reliably be applied. I haven't seen one. Unless... is willpower code for bariatric surgery?

Or cancer. Perhaps willpower is cancer. That is certainly an effective weight loss intervention. The ethics are a bit dicey, though.

As this is such a complex issue, and I agree there are many issues at play here, what would you consider the major social issues?

Socioeconomic deprivation is the main predictor of poor health and obesity. This can be due to lack of access to healthy food or cooking facilities. Other factors that can be related are mental health, physical health (obviously). Lack of time, lack of access to exercise.

Will power is not enough when the diet is wrong. Many can lose weight through will power, but gain it right back as will power is not strong enough. The brain's survival mechanism is very powerful and will overcome most people's efforts in time.
