      Ah - if this is a cluster-randomized design they aren't using the right statistical tests and they need to (likely) do much more adjustment for baseline factors. Interesting study but really hard to vet well. They should release data and dispel all doubt.

      YIKES!!! That's not a minor problem; it's both a really big, and really basic study design and statistical issue. Strongly suspect this isn't the only major problem...

      Deleting a thing until I am more sure what I am looking at. At best this is a cluster-randomized trial with a bizarre randomization scheme and the authors fail to account for the clustering (which completely wipes out statistical validity. It could be worse.

      It's definitely not a randomised trial, and I think it's just being misrepresented by people on Twitter. The authors even describe it as "prospective observational" study. The patients certainly weren't randomised and they don't describe randomising the wards at allpic.twitter.com/xT2pF6W5uK

      I think you may be right but this isn't Twitter misrepresenting an observational study. The authors stay in the abstract that patients were randomized to vitamin d or not. There may be deliberate misrepresentation here.

          There were a few English usage oddities in the paper which makes assessment harder. Didn’t understand how “baseline” VitD was higher in the treated over control group but figured this was because the intended meaning of baseline was not “prior to intervention”.
