So many to choose - a few good ones already listed. Good old confounding is well understood but people still ignore on a daily basis. So maybe I'd say novelty bias as I encounter it so much and it seems to be getting worse (I blame tech/startup mindset encroaching on healthcare)

Survivorship bias was among my favorites UNTIL a few years back when I found that measurement bias is far more pervasive and damaging. Because you firmly believe you are doing it properly, systematic readings are consistent, but you are way off the mark.
