Unfortunately, you don't seem to have read the purpose of the study. "to evaluate whether hydroxychloroquine could prevent Covid-19 in high-risk healthcare workers" Incidence of COVID-19 is the primary endpoint, not "severity of disease".

Endpoint was not "infection per se". The primary endpoint of this study was dx of COVID-19, a disease caused by SARS-CoV-2. Nothing precludes infection per se, not even vaccines. In fact, the endpoint of this study is the same as the primary endpoints in vaccine trials.

Although one could argue it adds little to the body of evidence due to wide CIs and *maybe* risk of bias (I have not checked it), the choice of primary endpoint was NOT inadequate. Hospitalisation, OTOH, would be more prone to bias (softer endpoint).

"The impact of covid is based solely on causing severe illness or death" is also probably incorrect. We could be interested in a treatment that effectively reduces viral replication (and thus viral load) as a means to reduce likelihood of transmission.

This is incorrect. Given that low severity cases have been modelled to be the driving force behind the pandemic, reducing/eliminating those would be of enormous benefit

Sure, but this is in the context of HCQ as a prophylactic. If it prevented mild disease, it could be an incredibly important finding even if it did not reduce hospitalizations
