This resonates with industrial incident investigation, where we determine proximate cause and root cause--to me, still an oversimplification. I identify proximate cause, contributing causes, and pimary cause. 1/2

Proximate, primary, and significant contributing causes are all given prominence in the report. Primary is merely the first place to devote corrective resources. Focusing on THE cause is a B&W oversimplification that causes much confusion.

It does get a bit more complicated in that Australia, the definition of a death from COVID-19 includes those people who PROBABLY had COVID-19. Otherwise, great explanation.
