I read about this recently. Apparently, the paper this comes from (or some other) does not aim to prove or disprove this, so it's not designed accordingly. For example, heart condition seems very important, and smoking leads to heart problems.

So you can conclude things like "heart problems very bad for covid", and "smokers with no heart problems doing great!", but overlook that many people have heart problems directly caused by smoking...
