- smoking is protective (!!!)
- being Black is risky, but not as risky as being poor
- hypertension NOT a risk factor
- watch out for poorly controlled diabetes

Actually looking at the paper the smoking element appears weak at best, and agreed could be a survivorship effect. It could also be an issue with collinearity with other risk factors (they mention ethnicity). In truth only had a quick look so far but don’t know ...
