Trials like this aren't rare though. It's one of those early stage "is this worth further study" trials, which frequently have all the flaws you mentioned. It's rare to see one published outside of oncology though, certainly in the NEJM.
I wouldn't base any decision farther than let's do a controlled trial next on it, because that's what its purpose is, and all it's really good for. Unfortunately, people outside the medical fields (and a few inside it) might see it as more, which is a problem.
