I'm basing it on both. The proportion of people with an unhealthy BMI but, for example, a healthy waist circ is very low

If you want to bring personal experience into it when I was a “normal BMI” I was malnourished, couldn’t walk up stairs, didn’t play sports, had gross hair, skin, and nails. Now at an “unhealthy” BMI I lift, I run, I glow. I’m not a “population” “at risk”, I’m me and I’m well.

I second that. BMI 25.0 here and perfectly healthy, with only 12% body fat - I could lose muscle and would be around 23. It's a fine indicator for regular people

I’m an overweight, middle aged, healthy, and active person. Each year at my physical, my doctor tells me she’d kill to have my metabolic profile. I think BMI is a factor to consider, not THE factor.
