The basic idea - nurses help people who go on insulin for the first time This is usually called "insulin titration" - helping people get their dose right (it can be very finicky!)

Problem is, nurses usually have to do this over the phone, because people need to be contacted every week or more often and coming in to the hospital is a big ask!

This is an issue because a) takes a lot of time for everyone b) missed calls happen a lot on both sides c) has to be done in allocated blocks of time (i.e. Monday morning) d) often hard to communicate blood sugars

So, we trialed using a mobile application where patients could communicate with the nurse asynchronously (whenever they wanted) and enter their blood sugars when they took them rather than over the phone

In short, we found that the app did better on a few measures, and about the same on all of the others In particular, it took up less time and there were fewer missed appointments

The nurses who used the app also MUCH preferred it (although the sample was very small) So, similar patient outcomes (or a bit better), and less wasted time all round
