This is well known for most drugs/supplements (not that they're always more effective for an outcome, but that they get absorbed better) since peristalsis/MMC Phase III slow down and the chyme from the stomach gets more time to be absorbed in the small intestine

I was also thinking about the impact of timings on medication adherence, although that might partially be because I just went to a talk on medication adherence

That's interesting. I've never really thought about adherence at certain times of the day, but I suppose it can affect it. Did info from any of the talks suggest that?

Nope, it was more speculative on my part. Just thinking about how people (particularly elderly at-risk people) manage their day and how the morning is often difficult in many ways

Lot of skepticism/questions about this study. Here is a one of many threads discussing the results. https://twitter.com/Ricky_Turgeon/status/1186991732522549248?s=19 … Cc @hswapnil

@hswapnil @thebyrdlab 🤔 is this was reignited the chromotherapy discuss yesterday? It seems like a deliberate effort is being made to obfuscate whether this is actually an RCT https://twitter.com/ESC_Journals/status/1186934294347964416 …

Yeah I'm a little suspicious here. I've also heard mixed responses on the adherence argument day vs night, though I havent read any of the evidence on it personally. That being said, i buy the mechanism i dont buy the magnitude

The absorption/timing argument also seems a bit weak - adverse events were the same in both groups, surely if there were different absorption patterns this would also be reflected in negative as well as positive events?

The absorption argument is weak in my mind because of the half life. Assuming someone has been on the drug awhile missing a dose would mean the end of the world here.

Absolutely true! If it truly causes more BP lowering, there should be some BP lowering adverse effects? Subendocardial ischemia & normal tension glaucoma are associated with nocturnal hypotension, as an example

I think it just depends on the drug and condition. Higher absorption doesn't always equal meaningful differences in outcomes, whether beneficial or adverse, and there's really not many crossover trials that are comparing different times to see the impact on relevant outcomes
