Basically, there's a complex relationship between the DTaP vaccine and mortality. Oddly enough, this was not seen in the DTwP vaccine, which some argue means that we should switch back

It's very complex, and very contentious. Some argue that the issue is purely due to socioeconomics (i.e. who gets the vaccine), while some argue that the increase in mortality is due to an immune problem caused by the inactivated vaccine that would be fixed with whole-cell DTwP
