      Also, it looks like they may have made their study into a useless trash fire statistically. Those are within-group comparisons, but they've got a control

      1) do not perform significance tests for within-group change in outcome(s). It is inappropriate to perform paired t-tests for within-group change, then use p<0.05 for one group and p>0.05 for the other to say that treatment was more effective at changing the outcome.

      Basically, it appears that they've compared before vs after in the CCI group and control, but not BETWEEN the groups, and said that because the differences in one group are significant and the other aren't, that the results are significant

      But that's a bit of a fraught statement - because of the nature of these statistical comparisons, it's hard to say what a before/after comparison on these within-group measures means

      This is especially problematic because this study wasn't randomized, so baseline differences between groups have a huge role to play in the eventual outcomes

      Given how big the differences are, I'd be very surprised if there wasn't still a statistically significant difference between the CCI and control groups, but my guess is that it would be significantly attenuated (i.e. not nearly as impressive)

      This is precisely what happened in DiRECT - because of the media attention on the study, a number of 'control' people lost a lot of weight, which made their intervention look much less impressive

          interesting! The control arm in DiRECT had almost twice the rate of remission (3 to 4%) as the "diabetes support and education" arm of Look Ahead (<2%) and 4x what is typically seen in community practice (closer to 1%).

          What if the "before study" diet was a lot worce in DiRECT/Virta then it was when Look Ahead was done, hence many pt with DM2 now would not have had it at the time of Look Ahead?

          Personally I expect https://www.diabetes.co.uk/forum/  and low carb has a lot to do with it. Anyone who does a little googling will find the forum. I found it due to the BBC doctor in the house TV programme telling me what to Google.

          Did @virtahealth not allow anyone in the control arm to move over to the intervention arm to control for this? They are afterall trying to prove how cost effective it would be be an insurance company to offer them as an option.
