If you collapse the data into months/quarters, won't you have the problem that you can't tell if the visits occurred after the event? (Unless, of course, the event is mortality)

I would've thought not necessarily? You'd lose some granularity ofc, but the event would "happen" after a period of either high or low utilization (previous quarter/month)
