You should read the blog. 1-2 in 1,000 cases catch SSPE, which is an infection of the brain and spinal cord. Impossible to prevent entirely, almost always fatal

Mostly attenuation, depends on the vaccine. If we're going to go into the immunology, it gets incredibly complex and individual to specific vaccines, because the way that they are crafted can be quite different depending on the disease
