      I heard a lot of excuses from product developers over the past week for why their devices do not seem to work in RCTs. Some say that their devices cannot be tested in a trial.

      Or things that are unethical (such as testing whether cigarettes cause lung cancer), or completely unfeasible due to logistics and/or finances. There are plenty of those

      Yeh but I was mostly talking treatments here, not risk factors and lifestyle influences. It's definitely true that there are things you can't test in RCTs, but this is almost never true for treatments themselves

      If you can intervene in a group of people, you can almost certainly intervene in half of them.

      The only exceptions I can think of involve testing ineffective treatments when we already have an effective one, like homeopathy for HIV or iridology for cancer diagnosis

          Education is an interesting one, as I have been repeatedly reassured by some of those researchers that you can do whatever you want to people's kids, but as soon as you start talking about only doing stuff to some of them, they can't cope for fear of their kid being disadvantaged
