There's also not much evidence to suggest that a return to ancestral living patterns would effectively reverse these diseases - even if it were possible

That paper reports on 4000 y "old" populations. That is not "ancient" from an evolutionary view. Moreover, agriculture started already 10-12 000 y ago, i.e. 400-500 generations. Homo sapiens evolved some 2-2.5 million y ago, i.e. 80 000-100 000 generations.
