They are able to get this result is by continuing to pretend that <40% carbohydrates by calorie is what is meant by a low carbohydrate diet.

I believe that is for reasons: 1. <40% includes lower and very low carb diets 2. There are very few/no RCTs on very low carb diets, so a systematic review would be problematic

I've seen similar lists before. Most of the studies aren't RCTs, and most RCTs are very small. So doing a MA becomes a bit pointless
