For 1 and 3, "But it was grass-fed and massaged daily!" doesn't cut it. Unless it's been regularly de-wormed, treated for virtually all disease, and kept at a perfect temperature - and EVEN THEN - it can still be a disease vector

Commercial sushi in the US is basically not. You'll find plenty of outbreaks of HAV or O157:H7 from green onions or cantaloupe or lettuce. But do a lit search on sushi-associated infections in the US.

Risk is extraordinarily small. Same is probably true with other undercooked fish sold commercially in the US - including (I'd bet) ceviche sold in US restaurants.

I would also steer well clear of any artisan soft cheeses if you are immunocompromised in any way or pregnant, especially raw milk cheeses.
