Interesting approach to test for reverse causation, by an analysis including the mediator (treatment adherence). Could have been formally tested in a structural equation model.

My first thought was that there may be reverse causation ie those with more advanced cancer more likely to use CAMs. However, treatment adherence accounts for most, but not all, the difference in outcome.

Yes I thought of that as well - It may also be more common for people with severe untreatable chronic conditions who also have cancer to use CAMs. But I do think that they've captured the key issue with this methodology
