If nothing happens, a lot of doctors could be up on criminal charges
You can easily identify if someone has accessed your record outside of your expected care. You can report this to the ADHA, who have an audit record, or you can simply go to the police

The first time an org accesses the record, (though I'm not clear if that's at the person level or document level) it's easy. Beyond that it's impossible. So, the user is somewhat protected from random groups trawling records, unless that's access from a group already flagged.

I suppose I'm arguing that it's not good enough but you're arguing that it's better oversight then individuals have now. I would argue that the connectedness of this system makes the exposure level much higher, which to me undermines the data control aspect you're advocating.
