One thing I am pretty certain of is that they've rounded everything. Creatinine showed a significant drop despite being exactly the same value in the control before/after

0.1 could be 0.05001 or 0.01499, and 0.6 could be 0.5501 or 0.6499. So you could have a Cohen's d of just under 2.0 while reporting identical numbers after rounding.
