I am old (73) and if I become delirious in hospital it is 80% likely that is caused by a doctor or a nurse. Health professionals need education and stop thinking physical restraints are the treatment for delirium

Great question. Used in acute care the CFS is meant to describe the patient’s state two weeks before they became ill. A defined baseline facilitates care planning: is recovery to that state likely? How navigable is return home if baseline achieved? How does carer feel about it?

Informative systematic review! Two thoughts: 1) Subtle nuances need measurement such as occult dementia (is my history accurate?) & frailty; 2) even if able to validate CDR to decrease imaging, de-implementation strategies will be needed

How do you decide which older adults need a head CT after a fall? Around 1 in 20 will have intracranial bleeding.

Delirium is under-reported in claims data but when cases are identified delirium was assoc with increased mortality up to 12 months after ED discharge. Nice work team.

Excellent review of the extant literature on nursing home-ED transitions of care. There’s less than you would expect given the critical importance of the issue. Glad & other talented investigators are on the case

Great reading from one of America's great physicians about how doctors (including emerg docs) can ALLOW patients to have the kind of care they want -- although many things about our system can make it difficult to do:
