I think that rates below "affordability" and "mental health care shaming", but it's not an insignificant factor

Yeah, fair. Affordability is certainly a major blocker for more people. But this isn't unrelated! One of the difficulties of affording therapy is that you have to shop around a lot before you find a therapist that is worthwhile, so there's a really high up front cost.
