Dec 10, 1963: I flew NF-104 2x: 2nd flt over 118,000'-re-entered in flat spin. Rode it down to just under 6000' & punched out.Rocket seat got caught in shroud lines,fell on me,cut me & put my face on fire.I opened face mask to cut off 100% oxygen.
