      Could this be a result of the Tesla coming across a scenario it’s never seen in the millions or billions of miles of driving data (a semi-trailer traversing a highway)?

      maybe not, given that the first known autopilot-induced fatality had a very similar cause.

          Maybe it didn’t “learn” from that accident? I’m thinking a semi-trailer traversing a highway is probably rare (I’ve never seen one when driving in California for many years!)
