As many knows now ACE2 is a receptor for #SARSCoV2 & part of RAAS. ACE blockers are common anti-hypertensive drugs. A important question whether ACE blockers increase risk for #COVID19? 3 papers answered this question in short please don't stop your anti-hypertensive drugs !!!

Background. ACE converts Angiotensin to Angiotensin II and ACE2 converts Angiotensin II to Angiotensin 1-7 in lungs & others tissues. This is part of the Renin Angiotensin Aldosterone homeostasis system (RAAS). #SARSCoV2 interferes in the RAAS with ACE2 https://www.nejm.org/doi/full/10.1056/NEJMsr2005760 …pic.twitter.com/Bp4vKJAP0P

Important family of anti-hypertensive drugs inhibit the RAAS system such as ACE blockers or Aldosterone receptor blockers (ARBs) #SARSCoV2 might interfere with action of these drugs by binding to ACE2. Hypothesis is ACE or ARBs increase ACE2 expression => increase #COVID19 risk !

An important question is ACE or ARBs increases #COVID19 mortality and/or morbidity. Please remember millions of patients take these anti-hypertensive drugs. To answer this question 3 papers in the @NEJM came out today from 3 different cohort of patients. Let's have a look.

First paper. Observational data from 169 hospitals in Asia, Europe, and North America -> 8910 patients -> 8395 survived. It shows preexisting cardiovascular condition risk factor for COVID19 but importantly not for ACE, ARBs or antihypertensive drugs !!! https://www.nejm.org/doi/full/10.1056/NEJMoa2007621?query=featured_home …pic.twitter.com/ymmr7BT9Kb

The second paper examines 6272 #COVID19 cases in Italy. They show ACE & ARBs were more frequently used in #COVID19 patients due to cardiovascular risk factor but again no evidence between ACE & ARBs blockers & increase #COVID19 morbidity & mortality https://www.nejm.org/doi/full/10.1056/NEJMoa2006923?query=featured_home …pic.twitter.com/1YTsYHezal

The third paper examines a cohort of 12,954 patients in New York and again found no evidence for an increase risk for #COVID19 with ACE, or others RAAS anti-hypertensive drugs. In fact they found a more protective role for beta-blockers https://www.nejm.org/doi/full/10.1056/NEJMoa2008975?query=featured_coronavirus …pic.twitter.com/dBsxpIXXeb

What all of this means? Cardiovascular diseases are co-morbidity such as coronary artery diseases are risk factors for #COVID19 => not surprising More importantly taking anti-hypertensive drugs do not increase risk for #COVID19 Keep taking your anti-hypertensive tablets !!!
