Without getting into the weeds of gender identity, my point is that humans will behave in extreme ways for fictional ideas. So I don't think your original assertion is correct.

Feelings are real, though. Gender is ultimately a feeling. The fact that a trans person puts themselves through the ordeal of surgery is enough evidence that they are being compelled to do so by a feeling, which we refer to as their gender identity.
