Sure, there are many factors, but some are much more important than others. I'd say Pakistan's surge was largely due to rising tensions in Kashmir as well as it's status as the main stronghold of LeT and AQ & Taliban in the aftermath of the war in Afghanistan ("AfPak").

I need to see their criteria for definitions and where the set parameters- because to not have Afghanistan move up during the Soviet Afghan war is pretty glaring

Indirectly, to be sure. But the US withdrew in 2011, and that's when the explosion in terrorist events really took off. Another way of looking at it is that Saddam, and then the USA, sat on a powderkeg of ethnic conflict in Iraq that finally went off when neither was left.

...with a big helping hand from SA and Iran, who helped support their own factions in Iraq - Sunni vs. Shia vs. Kurd is probably overly simplistic, there's also tribal, religious, etc. faultlines that also come into account.
