I mean the ethos of Apple products for like a decade or more has been to try to get rid of 'files' as a metaphor, so this shouldn't be surprising at all.
is file management on the iPad supposed to be a nightmare? "save to device" lands stuff who knows where and any app where you want to open a file navigates to ??? where you only see 10% of your files also you can't open zip files? why is this such a bad user experience

Exactly this. Somebody in Cupertino decided that "Files" were a bad user experience, so they're intentionally not part of the iOS vocabulary. Corollary: if you want to work with data, don't use iOS. Corollary 2: Or Android, but you didn't ask about that.

I feel pretty certain that Ipads were designed to be consumption devices. So general computing tasks and concepts that aren't consumption-focused were originally just prohibited. It's gotten a bit better - now they're at least enabled and tolerated.
