A footnote:
1. The avg duration of a clinic visit (return) w/ the doctor is 7 minutes so the EHR is >2X
2. Much of the 16 EHR mins is during the visit which disrupts the patient-physician relationship
3. The 16+ mins is "active time" the way @Cerner tallied (it's actually worse)

Why doctors dislike EHRs so much yet do nothing about them? They are main users of EHRs. Patient has no influence. Is there anyone other than doctors who can force a change in EHRs? At providers, EHR selection & Implementation committees are always full of doctors.
-
I don't think it's a matter of physicians not pushing for a change. It's the broken healthcare system, hospitals and insurance carriers that put up strongholds against a positive change that can be made.

EHRs are BILLING systems influenced by insurers and government, bought by health system admins and foisted on providers. They are NOT designed to be true health records. They are all like this, no opportunity for doctors to pick a good one.

If govt is going to mandate EHR use, why not mandate the code be open source? At least open API for mods or integrated apps. I know nothing about coding but this must be possible, right?
-
It’s possible. Just don’t rely on any of the legacy or incumbent EHRs to lead the charge. Platforms would first have to be unbundled than modular designed using APIs. EHRs are sorta like keurig that only allows authorized keurig cups to make coffee.

Dr Topol, could this be a low hanging fruit application of AI (ie: digital scribe systems, etc)? How do you see this helping us transform what EHR has become?!

we should be allowed to scan in quick check box / hand written notes on stable pts with no new info. That’s a great start.

As
#physicians we have the ultimate power to create change - we as a group have never chosen to wield that power - “too chicken” and too concerned at the consequences to our patients - we will continue to be abused - we will continue our spiral downward - UNLESS...Hvala.
