For EMR systems, I agree. But airgapped medical devices are still running XP in the wild. A lot.

The driver model that changed with Vista made it substantially harder to run embedded systems. And usability is factor, too

Not really, no. Some embedded code wasn't just difficult, but impossible, to run. And *nix is generally a no-go due to usability.
