Replying to @SPECFORD59 and
I tell my patients if someone is asking you to pay ridiculous amounts of money out of pocket for an unproven cancer treatment there’s a reason and it’s not that they somehow know something the rest of the international oncology community doesn’t. #bcsm

If we *really* want to change toxic workplace cultures in #medicine- addressing this expectation of constant availability to all people at all times is a great place to start. An important conversation worth having if we are to really revitalize our healthcare system.

Last week I was criticized for taking a vacation and being perceived “as unavailable" while I was out of the country that week. Here’s the thing: I *was* available, but in 7 time zones away. And we wonder why burnout is so prevalent in medicine... 1/

One of the least beneficial aspects of the electronic medical record and other digital remote-work tools is the effect they have on perpetual work and workloads. If I never really turn off my phone or email, do I ever really get a chance to relax and recharge?

Happy Friday to everyone except the insurance company who denied my patient with cancer the pain medicine they needed There are some urgent clinical scenarios that can become true emergencies as we wait for prior authorizations from “peers” who don’t see the suffering firsthand

The carboplatin/cisplatin shortage IS a big deal and it's nationwide. 😢

There is a national shortage of chemotherapy drugs called Cisplatin and Carboplatin. They are used for treating lots of cancers. This is a big deal.

I am on a never ending rollercoaster of pediatric viruses that I seem to have no immunity to from my kids. This is something they should prepare new mom's for. Like “hey guess how many times you’ll be getting Norovirus in the next few years!” Here’s some Zofran! #parenting

Please stop treating ER low disease like it is standard HR+ brast cancer. It's not. If I see one more oncotype ordered for This subgroup I’m gonna lose it... These are basal tumors. Basal.

Happy to report I was able to resolve the issue with my patient and a charity organization not wanting to evaluate her application for assistance during treatment. I do reiterate again though. All humans have breast tissue all humans can get breast cancer and we all need help!

People who volunteered for #clinicalTrials being presented at cancer conferences should get the equivalent of rockstar parking. Front row seats, personalized acknowledgments, standing ovations etc 🥰 More than anyone else in the room… you make the science possible. 🙏

Lee Mazurek had been on medication to keep his chronic disease at bay for nine years when a @Cigna employee flagged his case for review. The company estimated it could save $98,000+ if he switched parts of his treatment plan. Then came the denial. 🧵👇👇

I treat many patients with leptomeningeal disease from breast cancer every year and patients are always appropriately devastated to find out the are excluded from trials. We need to do better here. This is who NEEDs clinical trials. #OncTwitter #bcsm

Review of rand ph 3 trials of met breast cancer, lung cancer, & melanoma for eligibility of pts w/leptomeningeal dzs (LMD) showed only 8/244 (3.3%) specifically permitted pts w/LMD, none w/LMD-related endpoints, identifying an opportunity to advance field. ja.ma/3AdXj5p

95% of solid tumor oncology is OUTPATIENT. What you are seeing is end stage disease inpatient, which is awful, but before you judge think about all those years of good quality of life outpatient tx the patient had. This is especially important for residents to hear. !

This. Don’t pretend to think you know what oncology is like based on your INPATIENT experiences. That is not what MOST oncology looks like. #MedTwitter

I had the privilege of a student shadowing me today I think his main takeaway (other than my obvious caffeine addiction) was that oncology looks VERY different outside the hospital If any trainee is considering cancer care please rotate through clinic! ascopubs.org/doi/10.1200/JO

Attending on the inpatient service for 3/4 weeks this month. Have an ASCO poster to finish. Still seeing 13-15 outpatients a day while attending and both of my babies have fevers. What else should I add to my plate right now😣. Feeling a bit overwhelmed. #MedTwitter

Imagine looking at this data, replicated in multiple countries throughout the world, and thinking HPV vaccine is a "complex question" It's a *cancer preventing* vaccine with decades of safety data. Not complex at all.

I did not realize it was “May the 4th be with you” aka Star Wars day and saw a person dressed 100% as a Jedi with full sized Yoda stuffy walking into the hospital this morning on my way to clinic and just thought, hmmm well that brown cloak sure looks comfy. 😳#May4thBeWithYou

One issue with books about cancer when you are reading them for treatment advice is that the oncology field can change from week to week as far as new data, therapies and supportive care, yet books stick around for years. So keep that in mind when reading #OncTwitter

#bcsm Cancer PSA of the night. Books about cancer are nice to see someone’s experience or tips or opinion on cancer, but they are not the same as peer reviewed scientific texts, so take a step back and read them for what they are. And if something doesn’t sound right ask your onc
