Why do doctors (who are among top income earners in America) charge, for their services, amounts of money that so many people cannot possibly afford to pay?

This is a simplistic question. Ultimately we would have to charge zero for our work to make sure everyone could afford the care they need: some people can’t afford even a $70 office visit for a routine sick visit, for example. So what’s the bottom number we should charge?
