I did volunteer patient advocacy in late 90s & early 00s. Insurers' applications would ask about all conditions a person had ever seen a dr about, & then could kick a person off their plan when a person had a new diagnosis that might relate to an earlier long-forgotten dr visit.

I had a great doctor in the 80s who, after we discussed my depression, said, “now I want you to to know, I haven’t written anything about this in your chart because it’s none of the insurance company’s business. They’ll use any excuse to deny treatment”
