We do have a truly government run medical system in the US: the VA system. There, everyone-doctors, techs, nurses, are employed by the government. The British system is like this. I am not in favor of that type of system.

So, no, I’m not in favor of a VA type system where the government would be the owner/employer of all healthcare entities. I’m in favor of transitioning everyone to Medicare, remaining a contractor with an independent practice, vastly reducing overhead, saving money and lives...

I would do it in a step-wise approach which I can tell you about if you’re interested. Let me know if you want to hear more or if you have more questions. Thanks!

I don't mind reading about you plan. In regards to above What is the apparatus that all of the paperwork would be going to? I want to say streamlined because from what I can tell you want everything to be paid out by one entity and this would include the paper work process.

Also does your plan not include jobless? Because you mentioned paperwork being handled by your staff but if the end game is have one entity pay then I don't see how this is avoided?

Ok, this kind of blew up! 😉 I’m going to do my best to answer all the questions coming my way. If I miss any, I’m sorry! I’m not trying to dodge. For me, as a doctor, healthcare policy is my number one concern in politics. Transitioning would go like this:

      1. Shore up the ACA immediately by strengthening the exchanges and providing generous subsidies for those using them. Expand Medicaid to all states and expand Medicaid coverage. Eliminate the junk plans being put forth by Trump. This will protect people already covered...

      2. Expand Medicare to cover everyone from age 55 and up. Allow time (1-2 years) for current markets to settle and for employees in the insurance industry to change jobs. Keep in mind that *many* health insurance employees are nurses, who can transition...

Into direct patient care as more and more currently uninsured patients get access to care. 3. Continue to lower the coverage age for Medicare by 10 years every 1-2 years until all are covered. The step-wise approach will prevent massive job losses.

All would have to be written into the law so that future congresses would not be able to stop the transition. Obviously this would require a Congress that is heavily composed of Democrats.

Ahhhh I see. In order to do this we need an expedited Dem Congress to pass it. As for the medicaid part I don't know how we prevent GOP obstruction because they seem to always find a way.

Why do you necessarily think there would be job losses? Big insurance companies administer Medicare plans across the country. Is there any reason to think that the Medicare Administration wouldn't or couldn't do the same, just with more members?

The job losses would come from reducing administrative positions. Just as an example, in my small office, we would need 1-3 fewer people. Multiply that by 10’s of thousands of healthcare entities. Most SP advocates recognize this and have various plans to mitigate...
