Being from Spain, I also thought that high, frequent commissions was the usual thing apparently we've just not been really good at competitive banking and same has happened with Japan

Spanish here, depends on your bank. Specially if your salary goes into a bank account you should be not be paying fees (even CC), you should move to another bank in that case. Then there's other non-spanish banks you can use with no fees such as n26.

Depends on the country. Italy has an average cost of 80 euro, with cheaper accounts costing 20-30 per year. The interest is also basically zero (so you're only spending money to keep a bank account active), and you still pay bank transfers outside your bank.
