So let me guess, on a historic and academic level there are differences large enough to be called significant, but unless you're in very specific fields you will very unlikely ever run into a situation where the difference matters?

For the most part. おう is still pronounced as おうand not おお sometimes, however, such as in verbs like 追う、酔う、沿う, etc. Incidentally I cover this in detail in my Patreon series!http://Patreon.com/Dogen
