As it happens, more than half of our health care coverage IS public (Medicare; Medicaid; Tricare: VA; SCHP; BIA and Railroad Retirement System). Our problem, to some degree. is that we don't have A SYSTEM, we have SYSTEMS and people slip through the cracks.

The looming question can be summed up as: "How do you do "Employee Benefits" in the "Gig Economy." PPACA is an attempt but not an effective one. (High deductible insurance needs provisions for HSAs at minimum).
