If your idea's interesting, editors will often want to discuss details/angles: few editors have time to read a finished piece to reject it.

Lots of pubs have pitching guidelines online. Follow them, and keep it short:

Do go to commissioning editors, rather than ask writers to pitch on your behalf. If it's a good pitch, it'll stand up on its own merit.

If a publication have just published something on this topic, they're unlikely to take another unless there's a really imperative new angle.

Yes, without fail. Writers, I understand, PRs should really know that's not how it works.
