Why do some TV programmes list most actors, then say of someone in every episode “with...” instead. Eg It’s Always Sunny with Danny DeVito; Community with Ken Jeong?

Yeah I think that's it. Like after Anthony Stewart Head, Alyson Hannigan got that credit instead... which I assume is because American Pie got so popular in the mean time.

A very odd system, you're the top one but will be treated as an after thought

I assume in the UK it’s the same- that’s why in the later series of ‘Last of the Summer Wine’ Thora Hird always seemed to be credited: “Special Guest Appearance: Dame Thora Hird”. Also this might have coincided with less use of her character as she was less well over the years.

Oh, well, it’s mentioned on TV Tropes “Fake Guest Star” page: “Dame Thora Hird, Jean Alexander & Dora Bryan made a "Special Guest Appearance" virtually every episode for years on Last of the Summer Wine. They finally started being listed as members of the regular cast in 2002.”
