Yeah, it's a way for them to "top the bill" without actually being at the top (usually because someone else also has a massive ego and is more of a regular on the show).

I believe the with denotes that they are special in the cast. I remember hearing an actor say that once you get to a certain stature you should always ask for the "and" or the "with" in the credits.

Yes, that seems to be the answer, and apparently union rules have that baked in.

the sequence of credits is a big deal in the industry. "and" and "with" credits are special. i just googled it but I couldn't find a link that explains it. and in us tv of course, there's differences between regular/recurring etc
