The American experience is very much this. When you break your leg, you need to get yourself to a hospital (and with specialists) that take your insurance, or you will get a life ruining debt.
Haha yes I've had that text. They asked whether I would I recommend the A&E, as though I'd gone there for a pint and a ploughman's. It amused and bothered me to the point where I wrote a letter.

My favourite was Danny Bhoy who had to complete a survey to get a haircut, and asked what he hoped to achieve with his haircut answered a significant lessening in the tensions between Israel and Palestine.
