Also think this is party the problem with telephone consultations: used to trying to get off the phone as soon as possible, so never ask questions, when I would in person.

My GP has messed up my Dad's repeat prescriptions the last few weeks, we had them delivered from http://Well.co.uk and now the GP is sending to the Well Chemist down the road which is a pain the arse and they won't correct it.
