surely if that happens the polling clerks are required to record any instances and forward the reports for investigation?

I've never even heard of a friend-of-a-friend not being able to vote for any reason other than not being registered. And even then, that's rare. The ease of voting in the UK was one of the real sources of civic pride I had.
