does the fact that an aircraft engine exploded in mid-air yet landed safely and everyone appears fine give you more or less confidence in the design of the aircraft?

Neither. It's all about the maintenance of each individual aircraft and not the design itself. You can have a flawlessly designed aircraft that shouldn't ever fail but is poorly maintained, and you can have a somewhat crappy build of an aircraft that is meticulously maintained.

The question was about the design alone, not the design and operation. A product that must be perfectly maintained to avoid catastrophic failure has a less robust design than one that can withstand sub-system failure due to less rigorous maintenance.
