why is it that someone with 38 followers will write alt text for their images, but people with 100 thousand followers never do

Twitter doesn't ever prompt people to add it with an explanation. They could add a setting for making it required and a one time prompt when posting a tweet with missing alt text explaining it with the option to enable the reminder. It's really Twitter's fault at this point.
